Employee satisfaction survey templates: four free, ready-to-use questionnaires

Four free employee satisfaction survey templates — work environment, your manager, HR services and training — with original questions, how to run them and how to read results.

5 min read

Four free employee satisfaction survey templates, each on a topic people ask about most: the work environment, the direct manager, HR services and training. Every questionnaire uses original questions written for these templates, a clear five-point scale and two open questions. Each one downloads as a Word file, a PDF and a spreadsheet with a scoring tab, and comes with notes on how to run it and how to read the results without overreacting.

Which survey template do you need?

Choose the topic you need to understand — not every topic at once. A focused survey of 14 questions gets more careful answers than a long one about everything.

TemplateWhat it coversUse it whenWhy it is separate
Work environmentWorkspace, tools, safety, workload, respect, fairnessYou are planning changes to sites, facilities or ways of workingEnvironment problems are fixable fast, so they deserve their own read
Your direct managerClarity, feedback, fairness, support, follow-throughYou want to support managers, not just rate themManagers shape much of what people experience day to day
HR servicesResponsiveness, clarity, pay accuracy, confidentialityYou want to improve the service HR gives employeesHR is a service, and services improve when they are measured
TrainingRelevance, application, access, delivery, career valueYou want to know whether training changes workAttendance tells you nothing about whether training worked

How should you run an employee satisfaction survey?

Short, protected and followed by action — not a long annual ritual.

  • Keep it short: one topic, 12–15 questions, two open questions, under ten minutes on a phone.
  • Protect people: decide the minimum group size before launch — five is the usual minimum — and never report a smaller group. Before you write the invitation, decide whether the survey is anonymous or confidential, and say so.
  • Write in the language people read best. Offer Arabic and English, and do not ask people to answer in their second language.
  • Choose the timing: avoid the last days of Ramadan, the Eid weeks and your peak season. Keep the survey open for about two weeks, with two reminders.
  • Share results within 30 days, and say what will happen next, even if the answer is "we are still deciding".

How do you read the results without overreacting?

Look at denominators, distributions and differences between groups — not just the average.

  • Report the number of answers next to every result. Eight favourable answers out of twelve (67%) is a conversation, not a conclusion.
  • Use the share favourable (answers of 4 or 5) alongside the average. It is easier to explain and harder to misread.
  • Flip the reverse-scored items. Two questions in each template are worded so that agreeing signals a problem, and they are marked (reverse-scored). On those, favourable means an answer of 1 or 2. The spreadsheet reverse-scores them for you.
  • Compare groups with care. A gap of a few points between small groups is usually noise.
  • Read the open answers first. They tell you why the numbers look as they do.

What can a satisfaction survey not tell you?

What people would trade for, or what keeps them. A satisfaction survey tells you how people feel about their conditions this week, and those ratings drift with mood: in a hard year, scores fall across the board, whatever you did. For decisions about engagement, retention or your employee value proposition, add questions that make people choose between things they value. We explain why in why engagement scores drop in a hard year, and our GK Engagement Survey is built that way.

Frequently asked questions

Are these templates free to use?

Yes. Download, adapt and use them in your organisation. Please keep the credit line if you share them outside it.

Can I download the templates in Word, Excel or PDF?

Yes. Each topic page has a Word file, a PDF and a spreadsheet that scores the answers for you.

Can I combine questions from different templates?

Yes, but keep the survey short. If you combine, keep each group of questions together so you can report by topic.

What is the difference between a satisfaction survey and an engagement survey?

A satisfaction survey asks how content people are with specific conditions. An engagement survey looks at how much of themselves people bring to work and what drives it. Knowing how to measure employee engagement starts with that difference.

Do you have these templates in Arabic?

Yes. Every template is available in Arabic and English, on the page and as downloads.

Why are the questions original?

Several well-known questionnaires are licensed, and copying them without permission is not allowed. These questions were written for these templates, so you can use them freely.
